  • Granted access to medical records
  • Granted access to information related to billing and insurance

Without this document, your family may be prevented from making decisions related to your health and life-altering decisions if you should become incapacitated and unable to make these health-care decisions yourself. In these cases, it is often the court that decides what medical treatment you receive.
